I, the Member, have read and fully understand the rights and responsibilities stated in this Agreement and acknowledge that: (1) This Agreement cannot be modified or altered without an approved/written addendum by the Authorized Administrator and signed by the Member. (2) I understand that the Authorized Administrator reserves the right to deny coverage if the requirements outlined in this Agreement are not met. (3) I have read and fully understand the claims information and procedures. (4) I understand and agree with the subscription terms and auto-billing processes. (5) I authorize TruckClubTM to charge the card provided. (6) I understand that I am required to submit OEM coverage documentation, if applicable. (7) I am signing this Agreement voluntarily.
