I understand this is a automatically renewing subscription product and I authorize my payment method to provided to be charged at renewal.

I understand that there is a 30-day & 2,500 mile initiation period before ANY components within my plan become eligible for coverage.

Once your TruckProtect Enrollment Form completed and reviewed, a eSignature document will be provided within your account in the TruckClub Member Portal and emailed to you.
